Little Hearts Matter’s 2025 Strategy

Building a Brighter Future for Little Hearts Matter

At Little Hearts Matter, we’re proud to stand alongside families and individuals facing the challenges of single-ventricle heart conditions. In November 2024 our trustees approved our new two-year plan, Strengthening Foundations and Expanding Reach, that builds on the 30-year journey we celebrated in 2024. It focuses on strengthening our foundations in 2025 and reaching more people in 2026. The strategy is built on five key pillars to guide us. We are working to ensure every member feels supported, informed, and empowered.

Our Five Pillars of Change

  1. Revenue for Impact

Every pound we raise supports our vital work. To grow sustainably, we’re focusing on:

  • Building new partnerships with businesses, including Charity of the Year agreements.
  • Expanding fundraising events like community walks, firewalks, and dragon boat races.
  • Boosting digital fundraising through social media campaigns and crowdfunding.
  • Recruiting a new fundraising manager to focus on appeals and member engagement.
  1. Visibility and Engagement

We want everyone to know about single-ventricle heart conditions and the work we do. Our plans include:

  • Updating our website to make resources easier to access.
  • Running an Awareness Week campaign in 2025, led by adult members.
  • Sharing inspiring stories from members to highlight our impact.
  • Increasing social media engagement with more interactive and meaningful content.
  • Visiting hospitals and holding roadshows to connect with healthcare teams.
  1. Uplift and Empower

We’re here to lift up families and give them the tools they need. This means:

  • Publishing new resources like the “Taking Your Baby Home” guide and a transition booklet for youth moving to adult services.
  • Expanding digital tools for young people, including a our Youth Hub app and virtual spaces.
  • Hosting events tailored to different age groups.
  • Offering support for bereaved families with newsletters, events, and peer groups.
  • Updating resources to help with challenges like energy management and employment.
  1. Lead and Influence

We’re committed to driving change for better care and support. This includes:

  • Building on the success of our 2024 medical symposium with an online follow-up event.
  • Advocating for improved healthcare policies, focusing on lifelong care and benefits.
  • Partnering with researchers to explore quality of life and health outcomes.
  • Supporting changes in schools to meet the needs of children with heart conditions.
  • Helping create accessible medical resources for families who speak different languages.

5. Sustain and Thrive

We’re investing in a strong foundation for the future. Here’s how:

  • Training staff in areas like mental health first aid and safeguarding.
  • Moving to cloud-based systems and improving digital tools to work more efficiently.
  • Strengthening partnerships with the medical and CHD communities.
  • Recruiting more volunteers to enhance our member support services.
  • Finding ways to use resources wisely and save costs where possible, rolling out a digital-first approach while ensuring no-one is excluded.

Together, We Can Make a Difference

Our two-year plan is all about ensuring we’re here for every family and individual affected by a single-ventricle heart condition. Your support—whether through taking part in our educational events, donations, partnerships, or sharing our mission—will help us create a brighter future for all. Together, we can make sure that no one faces these challenges alone.
